Case Manager (full-time)
Alder Health Services is seeking a full-time Case Manager to provide support to individuals living with HIV/AIDS in South Central Pennsylvania and to individual with special medical needs accessing the primary care services of Alder Health. Successful candidates will exemplify the Agency's client-centered approach to medical supportive services and Alder Health Services' committment to helping all individuals accessing our programs acheive the quality of life they desire.
Case Manager Job Summary: The position works with individuals and families impacted by HIV and patients accessing Alder Health Services' primary care program who present with special medical needs which require additional support and assistance. All case managers at Alder Health Services work in a hybrid case management system which incorporates both time in the office and days off site meeting with individuals and families in mutually agreeable locations throughout the community, such as client homes, medical organizations, or other social services agencies. All services are provided using a strengths-based, client-centered service philosophy.
Case Manager Job Requirements: Applicants must meet the following qualifications to be considered:
•Applicants MUST have a BA/BS from an accredited college or university
•Applicants MUST be comfortable working with diverse populations including people living with HIV/AIDS, members of the LGBT community and those struggling with addiction.
•Applicants MUST have a valid Pennsylvania Driver's License
•Applicants MUST have reliable transportation. The position works throughout the community. Access to a car throughout the workday is a must to complete the assigned job duties.
•Bilingual (Spanish/English) preferred.

Patient Assistance Specialists (part-time)
The Patient Assistance Specialist is a new position at Alder Health Services. These part-time positions (2 based in Lancaster and 1 in based in Harrisburg) provide transportation and other services in individuals accessing the medical and support services of Alder Health Services. Interested individuals must be able to work 16 to 24 hours per week (8 hours per day for 2 to 3 days per week) assisting individuals accessing Agency programs access services throughout the community. Interested applicants must have high level of comfort working with diverse populations including members of the LGBT community, individuals living with HIV/AIDS, a clean driving record and a valid Pennsylvania Driver's License.
