HIV Case Management
Alder Health Services offers compassionate case management to individuals living with HIV/AIDS in South Central Pennsylvania. Our medical Case Managers help clients understand and access the many resources available to them. Our services are offered to anyone living with HIV that resides in any of the following counties: Adams, Cumberland, Dauphin, Franklin, Juniata, Lancaster, Lebanon, Mifflin, Perry, and York. We will assess your needs, and help you determine the best plan to address those concerns. Whether you need us on a short-term or long-term basis, we’ll be here for you.
Our HIV Case Managers Can Help You:
- Apply for assistance with medications or specialized care costs
- Enroll in entitlement programs (Medicare & Medicaid)
- Find a local food bank
- Connect with medical providers.
- Locate education and support resources
- Request housing assistance
- Maintain adherence to medication routines
- Connect with support groups
How to Enroll in Case Management Services
First, schedule an appointment with a case manager to assess your needs and the best level of care. We will ask about your health, the type of medical care you’re currently receiving, your living situation, and any social services you may qualify for.
You'll then work with your case manager to put together a plan to meet your specific needs. Whether it's a short-term plan to get you through a rough patch or a long-term plan that encompasses several different services, we can help you get where you want to be.
