Summary: Provides support and advances the mission and the programs and services of Alder Health Services through the coordination of services and programs for HIV+ individuals including the coordination of medical care and working in collaboration with HIV specialty medical providers by performing the following duties.

 

Duties and Responsibilities include the following.  Other duties may be assigned. 

Provides HIV case management services to a caseload in compliance with all the responsibilities and expectations of an Alder Health case manager.*

Schedules and conducts intakes with referred and/or assigned clients.*

Conducts individual clients needs assessments with all individuals served.*

Provides linkages to services and programs necessary to address client identified needs.*

Monitors client progress and updates individual service coordination plans accordingly.*

Coordinates with clients any available services at Alder Health including case management, support groups, and all client assistance programs.*

Participates in assigned client-related and volunteer trainings.*

Remains current on HIV treatment issues and community resources.*

Maintains a positive work relationship with all agency personnel and all assigned on-site case management locations and community partners.*

Ensures the highest level of confidentiality for all clients, client records, and client-related issues.*

Serves as an advocate for the needs of individuals impacted by HIV/AIDS.*

Adheres to all personnel policies.*

Sits for periods of time to complete case notes.*

Needs to occasionally move about inside the offices to access file cabinets.*

Performs other duties as assigned by the director of case management and support services.*

 

Supervisory Responsibilities:

Manages employees in the Case Management Department. Is responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Language Ability:

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

 

Math Ability:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

 

Reasoning Ability:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

 

Computer Skills:

To perform this job successfully, an individual should have knowledge of Microsoft Word, Microsoft Excel, email, and ETO for notes and to maintain file.

 

Education/Experience:

Bachelor's degree (B.A./B.S.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
