The position provides support and advances the mission and vision of Alder Health Services through the coordination of services and programs for HIV+ individuals including the coordination of medical care and working in collaboration with HIV specialist medical providers. The position assures the high-quality and patient-centered delivery of all collaborations between individuals in the community and Alder Health Services. Medical Case Managers recognize the unique needs of persons who are HIV+ and individuals who have traditionally been marginalized by the healthcare system. The position reports to the Supervisor of Case Management and Support Services.
